Pricing Challenges

Long-term care (LTC) insurance is among the most difficult products to price accurately. The long duration between premium payment and potential claim (often 20 to 30 years) creates enormous uncertainty in assumptions. Morbidity rates for cognitive impairment and functional limitations are difficult to project, as medical advances and demographic shifts make historical data less predictive. Lapse rates have been far lower than originally assumed, meaning more policyholders retain coverage long enough to file claims. Low interest rates have eroded investment income projections that were baked into original pricing, compounding the problem.

Risk Management

Many LTC insurers have experienced significant losses due to the combination of adverse morbidity experience, low lapses, and low interest rates. Regulatory rate increase requests have become common as companies seek to restore premium adequacy. Actuaries managing LTC blocks must regularly update assumptions, perform asset adequacy testing, and evaluate the sufficiency of reserves under various scenarios. New product designs (shorter benefit periods, inflation protection modifications, hybrid life/LTC products) attempt to reduce the uncertainty that plagued earlier generations of standalone LTC coverage.
